• We are conducting Sunday Masses at Christ the King Chapel located at Heaven’s Garden Memorial Park, every 4:00PM.

RE: POLICY GUIDELINES ON INTERMENT
DATE: June 2, 2009

Due to the changing cost of labor and construction materials, VTA Dev Corp is adjusting its interment fees at cost. This Memo replaces memorandum #2008-00105 of May 14, 2008 on the Required Documents and Fees for HGMP Interments:

• Photocopy of Certificate of Ownership (COO). HGMP lot to be used for the interment should be fully paid;
• If the lot is to be used by another person other than the lot owner, a Written Authority from the lot owner authorizing the use of the lot for the interment of the other person, accompanied by two valid Identification (I.D) and Photocopy of C.T.C
• Photocopy of Death Certificate of the Deceased;
• Photocopy of the Burial Permit;
• Accomplished Interment Order form (3 copies; for HGMP Staff, Office and Client);
• Payment for the interment fee in full, prior to the preparation days, with the following rates applies:

• REGULAR INTERMENT, three (3) regular days of preparation,Where the 3rd day as the actual interment day  
  P 13,000.00

• REGULAR INTERMENT, three (3) days preparation,Where a day of preparation falls on a Sunday or a Holiday
  P 14,000.00

• RUSH INTERMENT, with two (2) days preparation 
  P 16,000.00

• RUSH INTERMENT, with two (2) days preparation, Where preparation falls on a Holiday or a Sunday
  P 17,000.00

• In the case where there is more than one day that Falls on a Holiday or a Sunday on the preparation days,
  Additional fees applies on the additional special day of P 1,000.00/day.
